Richfield Health Center is a leading provider of long-term skilled nursing care and short-term rehabilitation solutions. Our 118 bed facility offers a full continuum of services and care focused around each individual in todays ever-changing healthcare environment.

Richfield Health Center serves Hennepin and Dakota Counties. We are located one half mile north of Highway 494 on Portland Avenue, next door to Richfield Towers. Fairview Southdale Hospital and the Mall of America are within minutes of our facility and several shopping centers and a grocery store are within walking distance. Our facility is Medicare and Medicaid certified.
